Registration

In the event my child is ill or injured and I cannot be reached, I give consent for Blumenort Preschool to refer my child to the family physician or have my child transported to the nearest hospital. I also understand that should my child be transported by ambulance, Blumenort Christian Preschool will have no responsibility for the costs involved.

I understand the fact that children will be under supervision at all times. I understand that there are several smaller rooms in the classroom, there may be times where my child is momentary out of view and under indirect supervision. I understand staff will frequently scan the room and ensure areas not visible are supervised. I consent to these conditions.

At Blumenort Christian Preschool, we go on several field trips each year. Parents will be notified of all field trips once the dates have been confirmed and will also get reminder notes near the date of the field trip. For all of our field trips, we will be taking the Blumenort Community Church bus. The following is consent for your child to attend all field trips for the year. All the field trip fees are included in your monthly fees. If you should choose to volunteer for any of the field trips, you will be responsible to pay your admission fees at the time for the field trip. I give permission for my child to attend all field trips for the school year. I also give permission for my child to be transported with the Blumenort Community Church bus.

Blumenort Christian Preschool supports and encourages the use of technology to allow our parents to be part of our classroom and observe their child's preschool experience through their personal devices using the "Lillio" App. These apps allows the teacher to take pictures and videos of the happenings and experiences in their classroom and post them for parents to see. Each class will have their own account and will only be available to the parents and/or guardians of each child in that class. This account is completely private and no access will be granted to anyone other than parents/ guardians and teachers of each classroom. As the parent or legal guardian of the child listed, I would like permission to participate in the Lillio app. As the parent/legal guardian, I will respect the privacy of others and not share or repost any pictures on this account; I will not screenshot any of the pictures: I will ensure any and all comments I post are respectful to all: I will not comment with any #hashtags; I will report any concerns, misuse or abuse of this account.

Any misuse or reposting to any public forum will result in your access to these technologies being revoked. As the parent or legal guardian of the child listed above, I give permission for this child to have their pictures and/or video posted on the "Lillio" App. These will only be posted on the Lillio App for each classroom to see. I understand that I may rescind the permission granted on this form at any time.

There will be a $25 registration fee due upon the acceptance of your registration. You will receive an email from the Lillio app once we have processed your registration. When you have received this email, you will be required to register your account and set up your payment information.
